If you are a new patient for Reva Sheppard

Just click on the New Patient Forms at the bottom of this screen, print out, then fill them out
and bring them with you to your appointment. It looks more time consuming than it really is, After the second two pages, you mostly just have to
sign the forms.  If you prefer you can fax them in to our office at:

with a copy of your insurance and driver's license and we can get started calling your insurance for approval.  If you wait and fill the New Patient paperwork in the office, please come in about 15 minutes early. This will enable us to get started with your session right away.
